Service Description

Appropriate reasons to establish care include medication maintenance and general health management & advocacy. Routine bloodwork is likely to be ordered during the initial intake visit. There are 3 options for this visit type: Initial Intake, 1 month follow up & 3 month follow up. If you are looking to establish care with John Meyer, FNP-C for medication management, please select the "New Patient Initial Visit" option. If you are a current patient of John Meyer, FNP-C and are needing routine medication refills, bloodwork, etc., select either "1 month follow up" or "3 month follow up" option. Please be advised that we do NOT prescribe or refill any controlled substance medications.
